Le 50e anniversaire de l'institution de la sécurité sociale a nourri nombre de réflexions sur la contradiction, sans cesse grandissante, apparue entre la volonté affirmée des pouvoirs publics d'assurer un égal accès aux soins pour tous les citoyens, et l'impossibilité, de plus en plus manifeste pour l'Etat, d'assumer seul la charge de cette ambition. Les tâtonnements successifs auxquels ont donné lieu des recherches de solutions montrent de plus en plus clairement la nécessité de définir, au préalable, les contours d'une politique globale de santé publique.D'autre part, dans un univers en plein bouleversement, médecine et médecins n'ont pour toute boussole qu'un serment d'Hippocrate, dont la valeur morale se situe, certes, hors de l'espace et du temps, mais auquel il est quand même difficile d'apporter des réponses adaptées à des questionnements d'une autre dimension. Et si c'est au politique, s'appuyant sur la réflexion éthique, qu'il faut aujourd'hui faire appel pour redonner au monde de la médecine - et au monde de la santé, tout entier - les bases indispensables à la construction d'une action au service de l'homme, c'est aussi - et surtout - le rôle de la médecine d'engager une telle action, pour redonner une dimension d'être à une profession revendiquant sa véritable vocation.

Abstract

Western medicine - especially in contrast with no-Western traditions of medical practice - is widely thought of as a coherent and unified field in which beliefs, definitions, and judgments are shared. Marc Berg and Annemarie Mol debunk this myth with an interdisciplinary and intercultural collection of essays that reveals the significantly varied ways practitioners of 'conventional' Western medicine handle bodies, study test results, configure statistics, and converse with patients. Combining theoretical work with interviews and direct observation of the activities and interactions of doctors, nurses, technicians, and patients, the contributors to this volume provide comparative studies of specific cases. Individual chapters explore topics such as the contested domain of fetal surgery in a California hospital, the construction of gender identity before transsexual surgery in Germany, and differences in the treatment and definition of pain by two clinics in France. 'Differences in Medicine' advances earlier studies on medicine's social diversity and regional variations to expose significant differences in the presumptions and decisions that affect patients' lives, and marks a dramatic development in both the study of medicine and in science studies generally. Revealing the ways in which the bodies and lives of people are constructed as medical objects by practitioners, technologies, and textbooks, this collection calls for and initiates new, more textured theories of the body and investigations in medicine and the practice of science. Marc Berg is a researcher at the School of Health Sciences at Maastricht University. Annemarie Mol is Professor of Political Philosophy at the University of Twente.

Abstract

In recent years, virtue theories have enjoyed a renaissance of interest among general and medical ethicists. This book offers a virtue-based ethic for medicine, the health professions, and health care. Beginning with a historical account of the concept of virtue, the authors construct a theory of the place of the virtues in medical practice. Their theory is grounded in the nature and ends of medicine as a special kind of human activity. The concepts of virtue, the virtues, and the virtuous physician are examined along with the place of the virtues of trust, compassion, prudence, justice, courage, temperance, and effacement of self-interest in medicine. The authors discuss the relationship between and among principles, rules, virtues, and the philosophy of medicine. They also address the difference virtue-based ethics makes in confronting such practical problems as care of the poor, research with human subjects, and the conduct of the healing relationship. This book woith the author's previous volumes, A Philosophical Basis of Medical Practice and For the Patient's Good, are part of their continuing project of developing a coherent moral philosophy of medicine.

Abstract

This engaging book examines what the Hippocratic Oath meant to Greek physicians 2400 years ago, and reflects on its relevance to medical ethics today. Drawing on the writings of ancient physicians, Greek playwrights, and modern scholars, each chapter explores one of its passages and concludes with a modern case discussion. The Oath proposes principles governing the relationship between the physician and society and patients. It rules out the use of poison and a hazardous abortive technique. It defines integrity and discretion in physicians' speech. The ancient Greek medical works written during the same period as the Oath reveal that Greek physicians understood that they had a duty to avoid medical errors and learn from bad outcomes.
